Decisions, issues and action items

Keep a journal of what was decided and why, log project issues and risks with a clear owner, and track the small follow-ups that come out of meetings.

Three kinds of record sit beside your tasks. A decision says what was chosen, why, and what else was considered. An issue is a problem, risk or blocker on a project, with people responsible for it. An action item is a small follow-up that has not become a task yet.

The Decision Journal lists decisions across your organization so you can find out later why something was done. It has no sidebar link yet. Open it at the /decisions address in your workspace.

  1. Open the Decision Journal.
  2. Use Search decisions…, or filter by project, type, status and date. Turn on Key decisions only to see the big ones.
  3. Click a decision to see its Rationale, Alternatives Considered, who decided it, and related decisions.
  1. Open a decision.
  2. Under Dependencies, choose Add Dependency.
  3. Pick the decision it depends on and choose Add. Both decisions then show the link.

Decisions are mostly recorded from an AI assistant connected to ProjectBrain, which can log a decision on a project while you work.

Each project has an Issues list for problems, risks and blockers. Open a project, find Issues on the overview, and choose All to see the full list.

  1. On the project’s issues page, choose New Issue.
  2. Give it a title and description. Say what could go wrong and what it affects.
  3. Set the severity and other details under Risk Assessment.
  4. Under Assignment (RACI), choose who is Responsible (does the work) and Accountable (owns the outcome). Leave it blank to assign people from the project’s roles.
  5. Review the similar issues shown, so you do not log a duplicate.
  6. Choose Create Issue.
  1. Open the issue.
  2. Choose Change Status to move it between Active, Resolved and Dismissed, with a note.
  3. When it is resolved, choose Capture Learning to write down what the team learned. Lessons can become guardrails.

ProjectBrain can also flag a Predicted Risk before anyone logs it. Treat it as a suggestion: confirm it or dismiss it.

The Organization Issues page shows issues across all projects with trends by severity, root cause and impact area. Turn on Systemic only to see problems that keep coming back.

An action item is a follow-up that is not yet a task.

  • On a standup, a follow-up has a Convert to Action Item button.
  • An AI assistant connected to ProjectBrain can save action items from meeting notes and turn them into tasks.
  • Your open action items appear in the Today’s Focus dashboard widget and in Focus mode.

Issues and decisions belong to a project. You only see them on projects you can see, so a walled project’s issues stay hidden from people who are not on it.
